First Choice Community Medical Services, P.C. (FCCMS) is a house call program designed to meet the needs of homebound older adults by delivering primary, transitional, and post hospitalization care in the Merrimack Valley and beyond. Here, we recognize the value of every person. We are guided by our commitment to excellence and leadership in providing skilled medical care for homebound older adults. Our committed, certified Nurse Practitioners and staff, allows us to maintain a quality of presence and a tradition of caring for our patients, which are hallmarks of our practice.

“The nursing team and I would like to extend our gratitude to Stephanie for the amount of support she gave us and Dorothy Serwon’s family yesterday during her passing here at HX. Dorothy was on service with a hospice company that was not responsive or supportive towards us, Dororthy, or her family. Dorothy was not properly medicated and therefor, was in pain. Stephanie took the time and adjusted medications, stayed with the family, and did not leave the facility until she was comfortable. Dorothy ended up passing moments later after being medicated properly. Stephanie truly went above and beyond for us and the family, and I needed to share this experience with you. From the first call to gather information, through the careful intake process, and by their prompt responses to every inquiry, the staff inspired our confidence. The nurse practitioner visits with Emily in her apartment at least once a month; is attentive to her physical, mental, and cognitive health issues; and collaborates closely with the nursing staff at the facility.
