First Choice Community Medical Services, P.C. (FCCMS) offers a specialized house call program focused on the unique needs of homebound older adults. We provide essential primary, transitional, and post-hospitalization medical care to communities in the Merrimack Valley and neighboring regions.

Our practice is distinguished by our commitment to excellence and our patient-centered approach. The heart of FCCMS is our team of certified Nurse Practitioners, who possess extensive training and experience in gerontological care. These skilled professionals conduct comprehensive in-home visits, enabling them to accurately assess, diagnose, prescribe medications, and monitor tailored plans of care for both acute and chronic medical conditions. By delivering care directly in the home, we ensure our patients can maintain their independence and enjoy the highest possible quality of life. Our consistent presence and deep-rooted tradition of caring are what truly define the FCCMS experience.

Dorothy was on service with a hospice company that was not responsive or supportive towards us, Dororthy, or her family. Dorothy was not properly medicated and therefor, was in pain. Stephanie took the time and adjusted medications, stayed with the family, and did not leave the facility until she was comfortable.
